Transformation of a case of tuberculoid leprosy in reaction to the lepromatous form.

چکیده
In an inquiry about the evolution of tuberculoid leprosy, I wrote in 1936 (7), that I had not seen a typical case of tuberculoid leprosy-characterized clinically by its well-defined lesions, bacteriologically by the absence of bacilli, histologically by tuberculoid structure, and immunologically by a positive reaction to lepromin-undergo transformation to the typical lepromatous condition with its characteristic features-diffuse, poorly delimited lesions, abundant bacilli, lepromatous histology, and negativity to the Mitsuda test. I asked for the opinion of other specialists on the matter, and added that if the majority of leprologists should assert that they had not observed this transformation, that fact would argue in favor of recognition of the tuberculoid form as a distinct type, separate from the neural and lepromatous forms. Up to the present time I know of no report of the transformation of a typical tuberculoid case, one which could be called complete in that it had been shown to possess all of the characteristics mentioned, into a frank lepromatous case, nor have I observed that occurrence myself until now. With the acquisitition of knowledge of the condition known as tuberculoid leprosy reaction, first described by Wade (12) and then by Schujman (6), Fernandez (3), Souza Campos(9) and others, cases in which the process of infection is accentuated, with abundant and marked lesions and with transitorially positive bacteriology, it was thought-and with some reason-that perhaps such cases might under some circumstances evolve to the lepromatous form.

Langerhans Cells in Skin Lesions of Leprosy
Background: Langerhans cells are important in the pathogenesis of leprosy. A recent study reported that these cells were almost absent in the epidermis overlying lepromatous lesions. Objective: To investigate a possible relationship between the number of Langerhans cells and the histopathologic spectrum of lesions in patients with leprosy. Methods: An immunohistochemical staining for S100 prot...

TUBERCULOID RELAPSE IN LEPROMATOUS LEPROSY Sir, Waters & Ridley ' recently reported 6 patients who had been lepromatous and who, after many years of chemotherapy and bacteriological negativity, were found on relapse to have upgraded to borderline-tuberculoid (BT) leprosy . In a related article,2 they reported the development of weakly posi tive lepromin reactions in some lepromatous patients tr...

Discussion Tuberculoid Hansen’s is a disease occurring in an immunocompetent individual with granulomatous reaction in the tissue. Usually no bacillus is identifiable. Nerve involvement is common with thickening of the nerve and numbness of the corresponding region. Leprosy caused by Mycobacterium leprae is one of the oldest diseases on earth. It has its earliest description in India and China ...
متن کاملConsequences of smallpox vaccination in leprosy patients.
This study illustrates the consequences of smallpox revaccination in 45 lepromatous, 28 tuberculoid, and 47 normal individuals. Results obtained with intradermal inoculations indicated that the patients with leprosy were associated with a relative anergy against the vaccinia virus, the anergy being minimal in the tuberculoid leprosy but marked in the cases with lepromatous leprosy. Major vaccin...

A 24 year old man admitted to hospital with borderline lepromatous leprosy was treated with rifampicin, dapsone, and clofazimine. After four months he developed a reversal reaction and the diagnosis was modified to borderline tuberculoid leprosy. The dose of clofazimine was raised and prednisolone added to the regimen without any symptomatic response. His condition improved dramatically after f...
